Your label must include
Required statement
“This product was produced in a private residence that is exempt from government licensing and inspection.”
Labels in at least 10-point font. Information goes on the package, or on a placard at point of sale for unpackaged items. Third-party vendors must display a placard: "This product was produced in a private residence that is exempt from government licensing and inspection. This product may contain allergens." Optional ODAFF registration number (format OKFFA-###-MMYY) may replace name, address and phone.
